Please mark "Fender Club" on the envelope. Your membership card will be sent to you once ready.

填妥申請表格，連同有效購物單據影印副本*，一併寄回「九龍尖沙咀廣東道三十號新港中心第一座十一字樓通利琴行市務部」收（信封請註明 "Fender Club"）或交回任何一間通利琴行即可。申請一經批核，會員咭隨即寄上。
